Update license to be consistent with the gate license

Congrats again on this work! I noticed the license mentioned in the gating procedure is different than the one attached to the model. This PR will update the model license so they are consistent.

I've also added the medical tag.

Thanks for taking a deep dive into our licensing language :) It's a bit complicated. The EHRSHOT License (which we're displaying now) is technically for our dataset. The model is licensed under CC-BY-NC 4.0. However we require users sign a DUA (the EHRSHOT License includes the core DUA terms in question) for accessing the model, similar to how PhysioNet provides both a DUA and a Data License when accessing their models. The language of the DUA is still being finalized but should be sorted shortly.

I don’t how best to denote what is effectively a composite access policy (DUA + license) in terms of structured metadata.

Got it! That makes sense. I've updated the model license to cc-by-nc-4.0 since it's currently the commercially permissible version. Perhaps specifying these dual licenses in the gate or the model card might be helpful to avoid confusion as I don't think we currently support multiple licenses.

PhysioNet's Data License and DUA are complementary, but the CC-BY-NC 4.0 license that you have assigned the model tells me I can share the model with whoever I want once I have access to it, provided I follow the CC-BY-NC 4.0 conditions: https://creativecommons.org/licenses/by-nc/4.0/deed.en

If you want to control access to the model and maintain an access list, you should change the license to something which closer matches the DUA.
